Thank you for choosing us to arrange your Tandem Student Parachutist medical examination. In order to help us complete the official forms, please answer all questions to the best of your knowledge and believe.
Skydiving is a risk sport where there is always a small but definite risk of death, injury or worsening of a pre-existing medical condition. The aim of the examination is to identify additional risks – to keep you safe.
Please complete our Tandem Parachute Medical Questionnaire and bring it with you for your examination, together with a
- Summary printout of your health record from your NHS GP
- A list of all current medication [should you be on any]
You need to bring a urine sample and any relevant medical information, hospital letters etc. for the examination. You also need to bring a form of ID, for example, your driving license.
The examination will take approx. 30 minutes.
